INSTRUCTIONS
- Download and review the “CCL Guidelines 2017”. Find the applicable pathway according to your qualification and experience as described on page 5 and 6 of the guidelines as well as respective forms.
- Download and print the respective PSIC Operator Credentialing Form of applicable pathway.
- Fill up and sign the Credentialing form.
- Upload the filled & signed form along with other required documents in the “Application Submission” section below. Click “SUBMIT” button at the bottom to complete the process.
- Three years of active Interventional experience after FCPS/MD Cardiology from a recognized CPSP Centre for FCPC cardiology training/Cath lab with CPSP accredited supervisor, Director Cath Lab. CPSP Centre accreditation certificate/CPSP Supervisor Certificate for Director Cath Lab should be part of submitted documents alongwith applicants experience certificate signed by Director Cath Lab. Separate covering letter from head of institute should also be submitted and uploaded.
