Cath Lab Registration Instructions

INSTRUCTIONS:

  1. Download and review the “CCL Guidelines 2017”.
  2. Download and print the PSIC Cath Lab Registration Form.
  3. Fill up and sign the Registration form.
  4. Upload the filled & signed form along with other documents in the “Application Submission” section below. Click “SUBMIT” button at the bottom to complete the process.
  5. Original filled form along with other documents to be sent by post to: Gen. Secretary PSIC, Office # 1602, High-Q tower, Gulberg-V Jail Road, Lahore, Phone number 042 32317355 (MANDATORY).
